In accordance with IRS guidance allowing an alternative manner of furnishing statements, Coastal Marine Construction and its affiliate Harbor Island Equipment will not automatically mail Form 1095-C (Employer-Provided Health Insurance Offer and Coverage) to all employees.

Availability of Form 1095-C

Employees may request a copy of their Form 1095-C for the applicable tax year.

How to Request Your Form

To request a copy of your Form 1095-C, please contact us using one of the following methods:

When You Will Receive the Form

A requested Form 1095-C will be furnished no later than the later of:

• January 31 of the year following the tax year, or
• 30 days after the date your request is received

Important State Information

• Massachusetts residents: If you were enrolled in employer-sponsored health coverage, your health insurance carrier will provide you with Form MA-1099-HC, which satisfies Massachusetts individual mandate reporting.
• Union employees: Employees covered under a union multiemployer health plan generally receive their health coverage reporting directly from the union plan administrator.
